Ch 9, Pg 196, Underlying Provider Failed On Open Chapter 9, error on Open

I am on Chapter 9, page 196. I am trying to run the WinForms app. I get an EntityException at the point the 'customers' List is being created:

"The underlying provider failed on Open."

I have downloaded the example winforms app from the website, and I get the same exact error. I can retrieve table data from the SQL DB connection, but I just can't pinpoint the origin of the error. Here is the exception detail report from my project ---- does anyone have any ideas?

EF can't find your database. Check the connection string and make sure it's pointing to a valid database server and that you have attached my sample database.

Sorry for delay in reply. I didn't get a notification and just noticed this today.
